The idea that an animal mouth is cleaner than a human mouth is an urban legend and is false. Cats and dogs carry some very nasty bacteria in their mouth that can give you unpleasant infections if they were to bite you. However, a human mouth is not exactly sterile either, so a bite from a human is also likely to get infected. Moral of the story… don’t let your cat or your neighbor bite you.
I believe — to answer the question of how this cat got into such an exaggerated pose — that this picture was taken from the opposite side of a glass door, upon which the cat is pressing its paws. This is a famous cat shelter in Germany.
btw there’s some confusion as to mouth cleanliness. There are natural antiseptics in cat and dog spit, BUT they only target SOME bacteria. However the bacteria that the saliva doesn’t target are from around and between the teeth. Cats and dogs don’t brush their teeth. So while you will get an infection from being bitten by a cat/dog/neighbor, you’re highly unlikely to get an infection from getting licked by cat/dog/neighbor.

the paws may be covered in ‘cat spit’ but to a cat, it’s CLEAN. Cats clean themselves by LICKING their paws. Hence teh cat spit.
